For what it's worth, here's a video I made with my children to help train managers at my work in giving feedback. It wasn't the managers' first exposure to feedback. The video takes some things for granted. I work in a software shop so some of the situations are a bit specific. Still, it gives me a chuckle when I watch it and I thought someone might find it useful.
