Regarding rule #4, "call right away":

By the time we had crossed T's and dotted I's before extending an offer to our first choice candidate, it was already well past the end of the day. I was hesitant to call in the evening, for fear it would raise concerns that I'd be calling all hours of the day and night. My team's personal time is very important to me, because it's important to them...and I don't want to get off on the wrong foot there!

In the end I went with "call right away", and the following approach:

"I'm calling to extend an offer to you, etc etc... I apologize for disturbing you at dinner time, I know family time is important to you. I was just so excited to talk to you that I couldn't wait for tomorrow... I'm happy to answer any questions you have now, or you can call me back at a time that's more convenient for you and we can discuss further."

Calling when I was at home actually may have meant more than calling from the office during regular business hours. It seemed to make the candidate feel really welcome, and turned a regular family dinner into a celebratory event. I'm happy to report that my new hire starts in 2 weeks!
