You might be looking for...

In the review prep casts  there is a reference to a self appraisal form.  Mark and Mike assume that your company has one; they don't provide one.  It is a rare company that doesn't have one - even most small companies have them.  The form is less important the content.  And this cast  (http://www.manager-tools.com/2006/07/preparing-for-your-review-part-1-of-2) tells you have to gather it all.  If you subscribe to premium content, it's a handy reference for all the data.
