Hardcopy of One on One Form

In the One on One podcast part #2, it was mentioned that there would be a one on one template posted on the website.  This could be used to document the one on one discussions.  I can't seem to locate the template.  Can anyone point me there?

Any special thoughts/resorces around handling the challenge of managing hourly folks who work a days then afternoon then midnights schedule?  I'm thinking of starting with O3s twice per month (when they are on days and afternoon shift).  Thanks for the advice.

Scroll down, and you should see both PDF and DOC versions.  I use a variant of these, customized for my team.  A colleague (who is a bit too organized) uses individually-customized ones for each of her staff.  (Including name, position, current assignment, birthday, hiring anniversary...)
